Hospital Medicine Program
A hospitalist is a physician who specializes in caring for patients during their stay in a hospital. Our Hospital Medicine Program is staffed by board-certified, internal medicine physicians who are available 24-hours-a-day, seven days a week to provide coordinated, high-quality care for patients admitted to the hospital.
How does the Hospital Medicine Program work?
When a patient is admitted to the hospital, the hospitalist will contact the patient’s primary care provider, and inform him or her of the patient’s condition. At that time, the primary care provider will provide the hospitalist with the patient’s records and as much information as possible regarding the patient’s medical history. The process is similar to what your physician does when he or she refers patients to a specialist for another opinion or for additional tests and treatment.
Throughout the patient’s stay in the hospital, and after his or her discharge, the hospitalist will keep in touch with the primary care provider, making sure he or she is informed about the patient’s condition.
Why is a Hospital Medicine Program good for my care? By working with the patient from the time of admission to the hospital, the hospitalist can better determine when tests should be run, be there to promptly interpret those test results and personally check on the patient more frequently. Being on-site, hospitalists are able to coordinate and consult with primary care providers and specialists, better monitor a patient’s progress, and are more readily available to patients and their families to communicate about the issues at hand. By directing the overall patient care process, the hospitalist can coordinate a more timely and efficient hospital experience ultimately leading to a better outcome.
