Care Management

The role of a care manager is to assist patients in the appropriate use of the healthcare system by acting as facilitator, coordinator, advocate and educator.

Care Management begins when a patient is holistically evaluated, looking at physical, psychosocial and emotional needs. If the care manager determines that further care management intervention is needed, goals are established with the patient and their family and monitored routinely over time. This process can take place immediately or over a long period of time.

The care management staff assists the patient and their family by helping to identify barriers to wellness; supporting and guiding them through episodes of illness, and linking them with needed resources. Advocacy, education and counseling are the three primary responsibilities of a care manager. The care management staff works in partnership with the patient and their family while respecting their rights and confidentiality to achieve wellness and better quality of life.
